Maintaining good oral health is crucial for overall well-being, especially for older adults. However, for seniors residing in long-term care facilities (LTCIs), oral health is often overlooked, leading to significant health issues. Poor oral hygiene and limited access to dental care contribute to this problem, compounded by the fact that healthcare workers in these facilities may lack adequate training in oral care assessment and practices. To address this critical gap, a new Oral Health Assessment Tool For Long Term Care has been developed and rigorously validated. This article delves into the importance of oral health for the elderly in long-term care, the development and validation of this essential tool, and its potential to revolutionize oral care practices in LTCIs.
The Critical Need for Oral Health Focus in Long-Term Care
The global population is aging rapidly. By 2030, it’s estimated that 1.4 billion people will be 60 years or older, a number projected to double by 2050 [1]. This demographic shift underscores the growing importance of addressing the health needs of older adults, and oral health is a key component of overall health that often gets insufficient attention.
Older individuals are particularly vulnerable to oral health problems, ranging from cavities and gum disease to tooth loss and even oral cancer [5,6,7]. In fact, studies show that a significant percentage of older adults experience untreated dental issues and avoid regular dental check-ups unless a problem arises [3,7]. This neglect is even more pronounced in LTCIs, where residents often face physical limitations and are more reliant on healthcare workers for their daily care [5,6,19].

The consequences of poor oral health extend far beyond the mouth. It’s strongly linked to a range of systemic health conditions, including diabetes, heart disease, respiratory illnesses, nutritional deficiencies, and even neurodegenerative diseases [6,7,8,9,10]. Oral problems can lead to difficulties in eating, speaking, and maintaining adequate nutrition, significantly impacting an individual’s quality of life and overall health [9,10,14]. Recognizing and addressing poor oral health in older adults, especially those in LTCIs, is therefore a critical healthcare priority.
Addressing the Gap: An Oral Health Assessment Tool for Long-Term Care
One of the major contributing factors to poor oral health in LTCIs is the insufficient training and resources available to healthcare workers responsible for resident care [27]. While educational programs aimed at improving oral care knowledge exist, their effectiveness in practice has been inconsistent [28]. Often, healthcare workers may not have a standardized, easy-to-use tool to guide them in assessing residents’ oral health and implementing proper oral care procedures.
Existing tools like the Oral Health Assessment Tool (OHAT) are valuable but may require specialized training that healthcare workers in LTCIs may lack [30]. This necessitates the development of a simplified, user-friendly oral health assessment tool for long term care specifically designed for these frontline healthcare providers [29].
To fill this void, researchers have developed and validated a new assessment tool intended to improve daily oral care practices and ultimately enhance the oral health of older residents in LTCIs. This tool is designed to be used routinely by healthcare workers involved in direct resident care, providing a structured approach to both oral health assessment and the delivery of oral care procedures.
Developing a Robust and Validated Tool
The development of this oral health assessment tool for long term care followed a rigorous five-step validation process:

Preliminary Tool Development: Based on an extensive review of existing literature and expert consultation, a preliminary assessment tool was created. This initial tool aimed to identify key factors hindering proper oral care and ensure effective preventative measures were included. The literature review encompassed studies published between 2013 and 2023, focusing on oral health and care practices in older adults and LTCIs. Databases like PubMed, MEDLINE, and CINAHL were searched using relevant keywords such as “oral health,” “oral care,” “assessment,” “checklist,” and “long-term care institution.”

Translation and Back-Translation: To ensure clarity and accuracy across different languages, the tool was translated and back-translated. This meticulous process involved native speakers proficient in both English and Chinese to guarantee the tool’s linguistic equivalence.

Expert Consultations: Two rounds of expert consultations were conducted. In the first round, experts reviewed the tool’s format, rating methods, and individual items, providing feedback on accuracy, relevance, and completeness. The tool was then revised based on this input. A second consultation with a separate panel of experts further refined and finalized the assessment tool. These experts included dentists, geriatric nurses, nurse educators, and dental hygienists, ensuring a multidisciplinary perspective.

Content Validity Testing: The content validity of the tool was rigorously assessed using the Content Validity Index (CVI) and Content Validity Ratio (CVR). Experts evaluated each item for appropriateness, clarity, and relevance. High scores for I-CVI, S-CVI, and S-CVI/UA were obtained, demonstrating strong agreement among experts regarding the tool’s content validity.

Pilot Study for Practicality and Feasibility: A pilot study involving nursing students was conducted to test the tool’s practicality and feasibility in a real-world setting. Two experienced assessors used the tool to evaluate oral care procedures performed by the students, assessing the tool’s clarity, ease of use, and comprehensiveness. Feedback from the pilot study led to further refinements, ensuring the tool was user-friendly and effectively captured essential aspects of oral health assessment and care.
This comprehensive development and validation process ensured that the resulting oral health assessment tool for long term care is not only scientifically sound but also practical and readily applicable in LTCIs.
Key Components of the Oral Health Assessment Tool
The validated oral health assessment tool for long term care is comprised of two key parts:
Part I: Oral Health Assessment: This section focuses on screening older residents for potential oral health issues. It includes items assessing various aspects of oral health, such as:
- Lips and Oral Mucosa: Evaluating color, moisture, and any abnormalities.
- Teeth: Assessing for presence of natural teeth, caries, and other dental issues.
- Tongue: Checking for coating, lesions, and mobility.
- Saliva: Observing the quantity and consistency of saliva.
- Dentures: Assessing denture condition, fit, and hygiene (if applicable).
- Oral Hygiene: Evaluating the presence of plaque and debris.
- Dental Pain: Identifying any reported or observed pain or discomfort.
Any abnormal findings during the assessment are flagged for reporting to the supervising nurse and subsequent dental consultation.
Part II: Oral Care Procedure Assessment: This section serves as a checklist to guide healthcare workers in performing systematic and thorough oral care procedures. It includes essential steps such as:
- Preparation: Gathering necessary supplies, ensuring proper lighting, and implementing infection control measures.
- Procedure: Step-by-step guidance on cleaning teeth and dentures, rinsing, and ensuring resident comfort and safety.
- Post-Procedure: Documentation, communication with the resident, and reporting any concerns.
This dual-part structure ensures that healthcare workers not only assess oral health comprehensively but also follow best practices in delivering daily oral care.
Impact and Future Directions
The development and validation of this oral health assessment tool for long term care represent a significant step forward in improving oral healthcare for elderly residents in LTCIs. The tool offers several key benefits:
- Enhanced Healthcare Worker Competence: Provides a structured, easy-to-use guide for healthcare workers, improving their confidence and competence in performing oral health assessments and delivering effective oral care.
- Improved Resident Oral Health: Facilitates early detection of oral health problems and ensures consistent, high-quality oral care, leading to better oral health outcomes for residents.
- Standardization of Care: Promotes a standardized approach to oral health assessment and care within LTCIs, ensuring all residents receive a consistent level of care.
- Versatility and Applicability: While designed for LTCIs, the tool can be adapted for use in other settings, including home care and community health programs, benefiting a wider population of older adults.
- Training and Evaluation: Serves as a valuable tool for training new healthcare workers and for ongoing performance evaluation, ensuring continuous improvement in oral care practices.
Future research should focus on wider implementation of the tool in diverse LTCI settings and longitudinal studies to assess its long-term impact on resident oral health outcomes. Further refinement could also explore incorporating technology, such as mobile apps, to streamline data collection and provide real-time feedback to healthcare workers.
Conclusion: Investing in Elderly Oral Health
Poor oral health in long-term care is a significant yet often underappreciated issue. This newly validated oral health assessment tool for long term care offers a practical and effective solution to improve the quality of oral care provided to older residents. By empowering healthcare workers with a user-friendly and comprehensive tool, LTCIs can take proactive steps to prioritize and enhance the oral health of their residents, contributing to their overall well-being and quality of life. It is crucial for healthcare administrators and policymakers to recognize the importance of oral health in long-term care and invest in resources and training to support the widespread adoption of such valuable tools. By doing so, we can ensure that older adults in long-term care receive the comprehensive and compassionate care they deserve, including the essential component of good oral health.
